By checking yes to the emergency treatment release in the SJCCCS online school registration system, a guardian/adult student authorizes the San Jose Conservation Corps Charter School to provide first aid treatment or obtain medical or surgical care including care rendered through the facilities of a physician or hospital while the student is in attendance at the SJCCCS program and its activities. This care may be given under conditions necessary to preserve my life, limb, or well being should an emergency arise. By consenting to this treatment, the guardian/adult student understands the school assumes no responsibility for expenses incurred.
