MEDIA RELEASE & CONSENT
The San Jose Conservation Corps Charter School (SJCCCS) utilizes a variety of media to communicate the mission and vision of the school including student interviews, the use of quotes, student art/written work, and the taking of photographs, movies, or videos. Participation in these activities are voluntary and direct consent is obtained whenever possible (such as in the case of individual interviews).
By signing the release, I grant the SJCCCS, their successors, and their assignees the right to record the image and/or voice and use the artwork and/or written work, on videotape, on film, on photographs, in digital media and in any other form of electronic or print medium and to edit such recording at their discretion.
I understand that my address and biographical information will not be made public. I further grant SJCCCS, their successors, and their assignees the right to use, and to allow others to use, these media assets on the internet, social media channels, in brochures, and in any other medium and hereby consent to such use.
I also hereby release the San Jose Conservation Corps Charter School and its agents and employees from all claims, demands, and liabilities, costs, and expenses whatsoever in connection with the above.
AUTORIZACIÓN DE USO DE MEDIOS Y CONSENTIMIENTO
La Escuela Chárter del Cuerpo de Conservación de San José (SJCCCS) utiliza diversos medios para comunicar su misión y visión, incluyendo entrevistas con estudiantes, citas, trabajos artísticos y escritos de los alumnos, y la toma de fotografías, películas o videos. La participación en estas actividades es voluntaria y se obtiene el consentimiento expreso siempre que sea posible (como en el caso de las entrevistas individuales).
Al firmar esta autorización, otorgo a la SJCCCS, sus sucesores y cesionarios el derecho a grabar mi imagen y/o voz, y a utilizar mis trabajos artísticos y/o escritos en video, película, fotografías, medios digitales y cualquier otro formato electrónico o impreso, así como a editar dichas grabaciones a su discreción.
Entiendo que mi dirección y mi información biográfica no se harán públicas. Asimismo, otorgo a SJCCCS, sus sucesores y cesionarios el derecho a usar, y a permitir que otros usen, estos recursos multimedia en internet, redes sociales, folletos y cualquier otro medio, y por la presente doy mi consentimiento para dicho uso.
Asimismo, libero a la Escuela Chárter del Cuerpo de Conservación de San José, a sus agentes y empleados de toda reclamación, demanda, responsabilidad, costo y gasto de cualquier naturaleza en relación con lo anterior.
